Alturas Health and Safety Protocol and Screening for COVID-19

Alturas is committed to maintaining a safe workplace for all our employees and all those who visit our facilities. Visitors must receive prior approval from Alturas management, and adhere to Alturas Health and Safety protocols. At our Alturas facilities, we have been operating as usual under precautions while following the CDC’s guidance on COVID-19.

At this time, onsite visit requests will be evaluated on an individual basis. Virtual audits remain available for Sponsors who wish to use this service.

Visitors may be asked to mask while on the premises. The mask must be worn properly to cover your nose and mouth at all times in Alturas facilities. Masks are available onsite, if needed.

Alturas guidelines may change according to regional infection rates, guidance by local health departments, ordinances by local government, or guidance by the CDC. Alturas reserves the right to refuse entrance to anyone.

Upon Arrival at the Alturas Facility:

  • You will be expected to attest to your health and to your willingness to follow Alturas protocols.
  • Frequently wash your hands or use hand sanitizer during your time at the facility.
  • Immediately inform Alturas if you develop symptoms of COVID-19 while onsite or within 3 days of being onsite at Alturas.

Alturas COVID-19 Attestation:

__I have not tested positive for COVID-19 in the last 10 days and am not awaiting COVID-19 test results.
__ I am not experiencing any COVID-19 symptoms listed by the CDC (fever or chills, headache, cough, shortness of breath, fatigue, muscle or body aches, new loss of taste and smell, sore throat, congestion or runny nose, nausea or vomiting, or diarrhea). __ I have not been in contact with someone who currently has COVID-19, has tested positive for COVID-19 in the last 10 days, or is awaiting COVID-19 test results.
__ I understand and will follow Alturas Health and Safety Protocols while on the premises.
